Guardianship – Important Information About Your Initial Appointment
Thank you for contacting our office regarding Guardianship. The link on the bottom of this page will take you to our initial intake form. This form allows us to be prepared for your meeting. Please complete the questionnaire as fully as possible and mail it to our office or drop it off with our receptionist. If this is impossible or inconvenient, please bring the form with you to your appointment.
During our initial meeting, we’ll discuss the guardianship and incapacity process, discuss less restrictive alternatives to guardianship, what assets would be included as part of a guardianship proceeding among other things. Please bring with you to your meeting any bank statements or other financial paperwork you may have for the alleged incapacitated person, copies of any medical records you may have related to their incapacity. We also find it helpful if you can put together a timeline of events.
We hope this letter has been helpful in preparing you for the initial consultation. If you have any questions, please write them down so that we can discuss them during our meeting.
